Terms of Service

On Call Primary Care provides direct primary care services on a membership basis. Our services include:

• Unlimited office visits with your primary care physician
• Same-day or next-day appointments when available
• Direct communication with your physician via phone, text, or email
• Preventive care and wellness services
• Chronic disease management
• Minor procedures performed in-office
• Coordination of specialist referrals

Our services do not include emergency care, hospital services, specialist care, or prescription medications (though we assist with prescribing).

Membership Fees: Monthly membership fees are due on the first of each month. Fees are non-refundable except as required by law.

Payment Methods: We accept major credit cards, debit cards, and ACH transfers. A valid payment method must be kept on file.

Late Payments: Accounts more than 30 days past due may result in suspension of services. Accounts more than 60 days past due may be sent to collections.

Fee Changes: We reserve the right to modify membership fees with 30 days advance notice.

Membership Cancellation: You may cancel your membership at any time with 30 days written notice. Cancellation takes effect at the end of your current billing period.

Appointment Cancellation: Please provide at least 24 hours notice for appointment cancellations. Repeated no-shows may result in membership review.

Termination by Practice: We reserve the right to terminate membership for violation of these terms, non-payment, abusive behavior, or other reasonable causes.

Not Emergency Care: Our services are not intended for medical emergencies. In case of emergency, call 911 or go to the nearest emergency room.

Not Insurance: Direct primary care membership is not health insurance and does not replace health insurance coverage.
